Description of the issue

I get the error: 'This item cannot be modified here because it is already being modified in a different window', even though there are no other windows open.
I have a very simple database, residing on the iPhone, designed to allow a remote engineer to add photo records (and later synch with our server using a connector file). I use a swipe panel for the engineer to enter a job No., a photo, and optional notes. I have a simple menu layout with a button labelled 'Add Photo' which creates a new record and switches to the slide panel layout. Panel 1 has one field, 'Job No'. Tapping this field brings up the numeric key pad. If the error/bug is present, the above mentioned message pops up the moment a numeric key is touched. There is no other window open at this point. I get the same message if I swipe to any of the 3 input fields (1 per panel) and attempt data entry.
The layout is set to "Save record changes automatically.
I have added 'commit record' trigger scripts for almost every action but to no avail.
It appears to me that there is a bug which locks the record erroneously (perhaps because of some random interruption during committing a record) which persists and prevents new records being created, even when the solution is replaced with an updated version.
I have not had this error occur when using the solution on my iPad (4th gen).

Steps to reproduce the problem

Normal use eventually causes the problem to surface.

Workaround

The only way I can get the solution to work again is to power down and restart the iPhone.
Updating the solution by replacing the solution file makes no difference - the error appears at first data entry.
