Description of the issue

I working to design a medical database. using Filemaker PRO version 12. I've created a portal for medical record notes along with a time stamp and user that inputed. My difficulty is figuring out how to define a calculation or way to prevent in the "notes" field to prevent previous modification or deletion, but to be able to input new notes at a current or later date.

PS: I am new at the program if you have any suggestions on best videos/books/software would be great
